Filling them out prior to your visit is quick and easy and will save you additional time at your appointment! After you make your appointment on the phone or online, check your email for a registration link from noreply@ezderm.com to access our online portal and enter your details.
Check the spam folder if you can’t find it or just contact us and we can resend the invitation.
