Insurance
We participate in most insurance plans including Medicare. For plans requiring a co-pay or for cosmetic procedures, payment is required at the time of service. Co-payments and co-insurance are determined by your insurance company. Please click here for a complete list of participating insurance providers (Participating Insurance Providers). Please contact your insurance company directly with any questions you may have regarding your coverage as plans may vary.
Referrals
If your insurance company requires a referral from your primary care physician, please contact this physician prior to your visit. We are happy to supply relevant information you may need, but it is your responsibility to obtain the referral. If a valid referral is not on file at the time of your visit, you will be asked to sign a referral waiver on the date of the appointment or given the option of re-scheduling your appointment. You will be responsible for any services billed if the referral is not provided appropriately.
Missed Appointments and Cancellations
Missed appointments or short notice cancellations deprive other patients of available consultation times. Please give us at least 24 hours notice if you must cancel your appointment. Missed appointments or providing us with less than 24 hours notice for a cancellation is subject to a $50+ fee depending on circumstances. Surgical appointments that are cancelled with <24 hours notice or no showed are subject to a $200+ fee.
